Chihuahua: hair loss around eye

I have taken him to the vet already. After an anti inflammatory shot and long acting antibiotic it has not gotten better. He also gave him ointment and nexguard. I am hesitant to give him an anti fungal or a stronger antibiotic as he is otherwise healthy. I will get a second opinion but just thought I would put this out there. Thanks!
